In addition to GP consultations the practice offers a range of clinics and healthcare services:
Citizens Advice appointments are also available through the surgery. Their service can provide you with free, confidential, impartial and independent advice on any issue.
An Adviser is available by telephone every Friday morning. Please telephone the Surgery for an appointment. Alternatively, you can telephone the Adviceline on 0300 456 8390 (Monday to Friday 9am to 4pm, calls charged at local rate) for advice and help.
If you require any vaccinations relating to foreign travel you need to make an appointment with the practice nurse to discuss your travel arrangements. This will include which countries and areas within countries that you are visiting to determine what vaccinations are required.

It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
Some travel vaccines are ordered on a private prescription and these incur a charge over and above the normal prescription charge. This is because not all travel vaccinations are included in the services provided by the NHS.
To help us offer the appropriate advice, please fill out our Travel Risk Assessment Form, so that our nursing team can assess which vaccinations you may require. You must fill out a separate form for each patient needing advice. You can email this form to email@example.com.
Please fill in this form with as much information as you can. Once you have returned your form to the Evelyn Medical Centre, you can expect a text message from us within two weeks to let you know that your assessment form has been completed by a member of our team.
If you are due to travel within 6 weeks of submitting your Travel Risk Assessment Form, we cannot guarantee that we will have reviewed your request and have been able to see you for any vaccinations you may require. Therefore, you may wish to seek travel advice elsewhere.
Please be aware we cannot guarantee that any vaccinations that maybe required will take place at Evelyn Medical Centre, you may be advised to book your vaccinations elsewhere. Some vaccines are not available on the NHS, these are prescribed privately and will incur a charge.
