Repeat Prescriptions
If you are on regular medication which your Doctor feels happy
to replace without seeing you every time, the prescription
may be obtained by the following methods:
Online - at any time - see below:
Ring on either 01433 621557 or 01433 621524 between 08.00am
– 6.30pm Monday to Friday
You can use the repeat prescription box in Hope surgery for
your prescription request slip or letter.
You can also request a repeat prescription by fax on 01433
621 745
Minicom users Tel 01433 621 227
If you live within a mile of the Hathersage Chemist “as
the crow flies” you can either collect a signed prescription
from the Evelyn Medical Centre or request that the prescription
be sent to the Hathersage Chemist for collection there.

Order repeat prescriptions on the Emis site and follow on screen
instructions.
You will need your:
Practice ID number
Access ID
Password
In order to book prescriptions online, you must have previously
obtained the necessary access codes.

There are two registration screens.
Screen 1 details - Your Registration Information
It's very important that only genuine patients are granted internet
access. The best way we can ensure this is to issue patients
with a Registration Letter, which can only be obtained in person
at your Practice. You cannot create an account without one.
The registration letter will give you your:
PIN number
Practice ID number
Access ID
NHS number
The information you enter on the first screen enables the system
to check your identity when you log on.
Screen 2 details
This screen is for existing users to sign in, you will need:
Practice ID number
Access ID number
Password
"ATTENTION!" When
making an appointment on line, the LOCATION of the surgery is
given below the Doctors name - HOPE or HATH (HATHERSAGE), The
LOCATION of your appointment WILL NOT SHOW on a printed confirmation.
Unfortunately this section of the Website is beyond Practice
control.

If, for any reason, you need your medication urgently.

If the Hope surgery is not open, then please listen to the
out of hours message and telephone the number given.
