We now offer patients the
use of a secure web site on the internet to:
· View, book and cancel appointments
· Request repeat prescriptions
Your online appointment is for a 10 minute appointments with
the Doctor. If you need a Medical Examination for a Job, Insurance,
Sporting Activity etc. do not book online. If you have any
doubt about the length of time needed for your consultation,
or if it would be more appropriate to see a nurse eg for contraceptive
advice or a smear, please phone our office on 01433 621 557
where a receptionist will help you book a relevant appointment.
"ATTENTION!" When
making an appointment on line, the LOCATION of the surgery
is given below the Doctors name - HOPE or HATH (HATHERSAGE),
The LOCATION of your appointment WILL NOT SHOW on a printed
confirmation. Unfortunately this section of the Website is
beyond Practice control.

Emis Access Registration - How to create
your account
You must be a fully registered patient and you will need
a letter from us containing your personal access codes. To
get this letter you will need to come to Reception and ask
for the “Registration for EMIS Access” your personal
details contained on the surgery computer will be checked
with you and you will be given a leaflet explaining how Emis
Access works and a letter which will supply you with the information
needed to Register with Emis Access.
Security Notice
It is your responsibility to ensure that your Emis Access
details are not used by anyone else. Any misuse of the service
will result in your access being withdrawn. If you lose your
security details please contact us straight away and we can
issue you with a replacement.

In order to book prescriptions online, you must have previously
obtained the necessary access codes.

There are two registration screens.
Screen 1 details - Your Registration Information
It's very important that only genuine patients are granted internet
access. The best way we can ensure this is to issue patients
with a Registration Letter, which can only be obtained in person
from our Practice. You cannot create an account without one.
The registration letter will give you your:
PIN number
Practice ID number
Access ID
NHS number
The information you enter on the first screen enables the system
to check your identity when you log on.
Screen 2 details
This screen is for existing users to sign in, you will need:
PIN
Practice ID number
Password
When you have collected your Registration letter from the Practice
create
your account to register your information.
If you live within a mile of the Hathersage Chemist “as
the crow flies” you can either collect a signed prescription
from the Evelyn Medical Centre or request that the prescription
be sent to the Hathersage Chemist for collection there. >
If, for any reason, you need your medication urgently.
Ring the surgery on 01433 621 524 and explain your need.
If the Hope surgery is not open, then please listen to the
out of hours message and telephone the number given.
