New Patient Registration

Surgery.jpgIf you live within our practice area you are welcome to register with us; please visit our Hope site to register. 

Click here to check if you are in our catchment area.

You must visit Hope practice in person where we will give you information on our practice and ask you to complete
a registration form and a new patient questionnaire.
You must bring with you some form of photographic identification such as a passport, driving licence, NHS card, and a recent utility bill.
 
We will ask you to make an appointment to see one of our nurses where we can ask you about your health, complete our records and answer any health related questions.
If you are using Android or iOS tablets and mobiles you will need to download the Patient Access app found at the botton left corner of the sign in page on the Emis website.

How to REGISTER:
New Users - please read the following before attempting to register with this service.
You will need the Patient Access Registration Information Letter from the paractice to complete registration. It's very important that only genuine patients are granted internet access. The best way we can ensure this is to issue patients with a Registration Letter, which can only be obtained in person at your Practice. You cannot create an account without one.

There is a Register link lower down this page to take you to the Emis Patient Access Page, please read all of the following first.

Emis Patient Access Page
There are two registration panels. One, a green Register panel, the other a yellow Sign In panel.
With the letter in front of you, click the green Register button.
Answer Yes to the question

Fill in the registration details from your letter.
Click Next
Enter your surname, date of birth and choose a password. You will use this password to sign in.
Click Next
Enter your contact details (optional) and choose some security questions.
Click Next
You have now successfully registered.
Note down your User ID.
You can now use this to Sign In along with your chosen password.

The information you have entered enables the system to check your identity when you sign in.

Sign In details
This yellow panel is for registered users, you will need to enter the following information each time you sign in:
User ID
Password ( chosen by you during the registration process)
